Daily Health Questionnaire
Parents who wish to drop their children into the Kids Club must attest to the best of their knowledge that the following items are true. Parents who decline to complete the screening will not be permitted to check their children into the Kids Club.
Today or in the past 24 hours, has your child or any household members had any of the following symptoms?
- Fever (temperature of 100.0°F or above), felt feverish, or had chills
- Cough and/or Sore Throat
- Difficulty breathing
- Gastrointestinal symptoms (diarrhea, nausea, vomiting)
- Fatigue
- Headache
- New loss of smell/taste
- New muscle aches
- Any other signs of illness
In the past 14 days, has your child had close contact with a person known to be infected with the novel coronavirus (COVID-19)?
Have you given your child any medicine this morning to lower a fever?
A “yes” answer to any of the above would mean your child would not have access to Kids Club today.
